YOUR FIRST VISIT:
As a new patient, please be at our office 20 minutes before your first scheduled appointment in order to fill out the new patient paperwork.

For your convenience, you can print our New Patient Forms and complete them prior to your appointment. Bring your filled out paperwork with you, or you can fax it to the office at 972-401-3230 prior to your appointment.

IF YOU NEED TO CANCEL YOUR APPOINTMENT:
Please let us know as soon as possible if you will not be able to make your appointment. We understand that things come up, but we request a minimum of a 24-hour notice so we can schedule another patient who may need to see the doctor.
